Phantom game stuck, and I can't import titles.

I am experiencing an unusual issue when importing games to one particular console. When I try to add games to Sony Playstation, they do not show when I navigate there. There is only one game that always shows, and when trying to open/play, it gives game cannot be found error. I have tried deleting the game in launchbox and on my sd card, but the same game keeps coming back. It never deletes. Mass importing vs individually adding a game doesn't make a difference. They never show. I've tried deleting the console and starting from scratch to no avail. I'm stumped.
