LED Blinky random buttons

I'm having some issues with setting up LEDBlinky.

I'm using:
    Windows 10
    Ultimarc Ultimate I/O
    Launchbox (paid copy)
    LEDBlinky (paid copy)
    
My panel is:
    2 players with joystick and 8 buttons each
    player 1 coin (5) and start (1)
    player 2 coin (2) and start (6)
    save state (F2), load state (F3), slot increase (F6), slot decrease (F7)
    select (enter) and exit (esc)
    pause (p), mute (F9), volume up (+) and volume down (-)
    
When I start 1942, I get the voice saying "1942", then "Fire" and "Loop" with the correct button colors
But it only lights player 1 start and player 2 coin.

How do I get the player 1 coin and player 2 start to light up?
