I can try to remake this template. Its been over 3 years now since I messed with these and the drive this was one crashed. It was one of the only drives I did not have being backed up for some dumb reason. I will see what I can get done for you.

Here are the 2 files. It was a quick put together as I have to head out of town for work. So these should fill the gap on these 2 for now for you.

There are some very slight differences as I could not remember drop shadow, gradient, and some positional settings I did in the original. When I return from my business trip I will try and sit down to see if I can refine the template more and then post it. Cannot promise any ETA. My time at home is short before next trips.
