LaunchBox is not saving downloaded images from Epic Game Store

You can always press F5 to refresh the grid image and see if that corrects. Given that the art isn't showing in the 3D box either though leads me to wonder if your image group priorities haven't been altered to not show EGS Posters. If you go to Tools > Options and look for Box Front Priorities, in that list is Epic Games Poster checked? If not that would explain why it's not being used for that purpose.

I did that - you can even see the refresh in the video, as well as that when I open the edit dialogue window again the images are no longer there.  I've repeated multiple attempts and the images never "save".  This is happening with at least 2 games, both of which are imported from the Epic Game Store.

What does the name of the other game look like (provide a pic of how it appears in LB)?

Reason I ask is maybe the image is not linking due to the fact that game name you are showing in the video has the Registered (®) and Trademark (™) symbols in the actual name  Dishonored® :Death of the Outsider ™

LB may be saving the image in the folder but the syntax it gives that image name is likely not matching the game. If you open the image folder is there an image for that game there?

This seems like a possibility.  The other game I also have on Steam, which doesn't have the issue, but also doesn't have the TM symbol:
