@d8thstar : Maybe something like this one I found and cleared background for. Has the same alignment as your other one.

i'm so close retro.  and my excel sheet will be release SOON to the wild, if anyone wants to use it.  here is an updated snip showing the add of original system/controllers

i also added above / below controller sections where the user can add notes in the back end and have them appear.
